![]() Registration Process There are two ways of registering for camp. You can choose the on-line method or the paper method. Please follow the instructions for your choice. |

On-line Registration Process If you fill out all the information, and come to the point where you need a credit card for payment, you can either enter the credit card information or email/call/fax Camp Heritage and give them the payment information. The information you have entered will still be in the database. Please be sure to include the last name, home phone and/or the account number to complete your registration. The Medical Healthy History Information can be completed on the web site and must be completed prior to the camper's arrival along with the "Consent to Medical Treatment and Authorization" form. If you are unable to complete this process on-line, please continue with the paper method. The original "Health History" form needs to be mailed or faxed to Camp Heritage prior to arrival. Paper/Postal System Registration Process If you register on-line, you DO NOT need to send in a paper application form. The following forms are for those who need paper application forms. To request one by mail, please call, fax or email Camp Heritage at the address below. If a camper wishes to attend more than one week of camp and is using the paper system, then a separate application must be completed for each week. |
